Understanding Your Benefits

Before your first session, we recommend calling the member services number on the back of your insurance card and asking the following:

  1. Do I have mental health benefits for outpatient therapy?
  2. Have I met my deductible for the year? If not, what is my remaining deductible?
  3. What is my copay or coinsurance for outpatient mental health services?
  4. Do I need a referral or prior authorization?
  5. Is Liz Calabrese (NPI: [insert]) in-network on my specific plan?

Out-of-Network & Superbill

If you have a PPO plan that is not listed above, you may still be able to receive partial reimbursement for therapy through your out-of-network benefits. A superbill — a detailed receipt with all required billing codes — can be provided upon request for you to submit to your insurer.

Call your insurance and ask: 'What are my out-of-network mental health benefits for outpatient therapy?' Many PPO plans reimburse 50–80% of the allowed amount after the out-of-network deductible is met.

Private Pay / Self-Pay

Private pay sessions are available for clients without applicable insurance coverage. Please contact Liz directly to discuss current rates and sliding scale availability.

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of the expected cost of non-emergency healthcare services, including mental health therapy. You can request a Good Faith Estimate at any time before or after scheduling your first session. If you receive a bill that is $400 or more than your Good Faith Estimate, you have the right to dispute it. For more information, visit cms.gov/nosurprises.

Frequently Asked Questions: Insurance & Fees

Q: Do you accept Medicaid or Medicare?

Embodied Living Counseling does not currently accept Medicaid or Medicare. If you are seeking therapy covered by Medicaid, resources like Open Path Collective or your state's mental health authority may be able to help connect you with low-cost providers.

Q: What if my insurance changes mid-treatment?

Please notify Liz as soon as possible if your insurance changes. Depending on your new coverage, you may be able to continue with your existing plan or explore self-pay or out-of-network options. Coverage changes do not need to interrupt your care.

Q: Is consultation for mental health professionals covered by insurance?

Professional consultation services are billed directly and are not covered by client insurance plans. Consultation fees are available upon inquiry.
